Download the "Subscription Update Form" below and follow the simple instructions. This is the only way to change your subscription. Do not email or call the office to cancel or make changes. Once you fill out the form below and fax it back to the office, a staff member will call you and confirm the changes. In the event of a cancellation, you will be issued a Cancellation Authorization Code. If you do not have this code you will continue to be billed.
