Download the "Product Return Authorization" form below and follow the simple instructions. This is the only way to return a product. Do not email or call the office to return and do not just send the product back. No refund will be issued.  Once you fill out the form below and fax it back to the office, a staff member will call you and give you a Product Return Authorization Number. If you do not have this number you will not receive any refund. This is necessary to prevent fraudulent returns of unlicensed material and to ensure that your refund is processed in a timely manner.
